Emmaus offers a private and casual setting where our clients simply come in and take a seat without checking in with a front office. We believe this is a less stressful and warmer environment. Please note that many of our locations do have stairs so if this is an issue, please call our Care Coordinator to schedule at a location that is better suited to your needs.
If you do not have time to complete the registration packet ahead of time, please plan to arrive 20 minutes prior to your appointment to complete in our lobby.
WHAT TO BRING
Please bring your completed New Client Registration Packet and your Therapist’s Disclosure Statement to your initial appointment. In addition, please review our Notice of Privacy Policies and you may click on this link to print a copy for your records.
WHAT TO HAND TO YOUR THERAPIST
- Completed & signed New Client Registration Paperwork & Therapist’s Disclosure Statement.
- Insurance card, Co-payment and Photo Identification (if applicable).
- Simplify making co-payments using our Credit Card Authorization Form.
Please provide a 24-hour notice for all appointment changes and cancellations. Lack of required notice will result in an $80 fee to your account which will not be billed to your insurance.
